PARENT CHECKLIST


A completed registration consists of the following criteria: registration fees ($600) paid in full, and submission of all of the
required documents. 

Also required: Mandatory Fundraiser equaling $200. 

If you will be using league supplied Helmet and Shoulder Pads there is a $200 Deposit Required.

Failure to do so, will result with the player not participating in practice and games until all items are met.

THE FOLLOWING DOCUMENTS MUST BE TURNED IN BEFORE JULY 1st

Copy of Birth Certificate

  • No baptism or hospital certificates will count. Must be the original birth certificate
  • Original Birth Certificate will be needed for Certification

Physical

  • Must be on the official Pop Warner Physical document, which we will provide you. 
  • Must be in color, no copies of the form will be accepted. 

Report Card

  • Must show the child’s entire year’s grades

Most Current Utility Bill

  • Any utility bill, bank statement will work

MONETARY RESPONSIBILITIES

$200 Deposit for Equipment Usage

  • Parent must submit $200 cash as a deposit for usage of the league issued equipment.
  • Deposit is due on equipment handout day, (You will receive a Cash Receipt after your Deposit is collected and to get 
    your deposit back you must have the Receipt)
    .
  • Failure to do so, will result in the player not receiving their equipment.
  • Deposits will be given back at the end of the season or when equipment is turned in.
  • If equipment is not returned, and is lost or damaged, the deposit will not be returned. You must have the cash deposit
    receipt in order to get your deposit back. 

Registration Balances

  • All registration dues must be paid in full by July 1st, or the player will not be allowed to participate. 
  • Failure to do so, will result in the player not receiving their equipment and their uniform until balance is paid in full.

Mandatory League Fundraiser

  • To keep fees low for players, the league requires each player to sell 10 golf balls at $20 each.
  • The fundraiser winner will receive $1,000. The player selling the most golf balls will receive a $250 gift card. The team selling the most golf balls will receive a pizza party. 

Required Equipment to be purchased by Parent

  • Cleats
  • Mouthpiece with strap
  • Protective Cup
  • Chin Strap
  • Integrated football pants
